The Ministry of Labor has outlined the methodology for forming medical workers’ incomes under the new remuneration system

Kotyakov described the payment system. Thus, for all positions, regulators have provided differentiation by complexity of work and, based on this, have formed 25 groups by salary. The minimum value of the complexity coefficient is 1, the maximum is 4. Each group has its own value.
The head of the Ministry of Labor noted that assignment to groups will directly depend on the level of qualification for the relevant position. In this regard, the employee's salary may increase upon promotion and transition to a new group. In addition, the project presents an indicator "regional differentiation coefficient", which takes into account the difference in the cost of living depending on the subject of the Russian Federation.
"They also proposed a single calculation value - the base for establishing salaries. It is supposed to establish salaries based on this single calculation value, multiplied by the coefficients of labor complexity and regional differentiation," explained Anton Kotyakov.
At the same time, the Ministry of Labor has already systematized various compensation and incentive payments and created their unified lists. The relevant departments "came to a clear understanding" - all compensation payments must be regulated at the legislative level, and there should be no other similar payments.
"We want all public sector employees, regardless of the region, to receive a fair salary. Our task for 2025 is to model the entire system in pilot regions, but not implement it yet. In 2026, we will implement the system in individual territories, and in 2027, we will distribute the final version of the system throughout Russia," Kotyakov said, specifying that in 2025, the new system will be tested in 15 regions in various industries.
Industry regulators planned to launch a pilot project for a new system of remuneration for medical workers back in November 2021 in seven regions - Yakutia, Belgorod, Kurgan, Omsk, Orenburg, Tambov regions and Sevastopol. The launch was postponed several times, and in July 2022, the Russian Government suspended its implementation until 2025.
In 2024, Russian President Vladimir Putin gave the order to introduce a new system of remuneration for doctors in 2026 in order to balance the situation with doctors' salaries. Currently, depending on the region, it can differ several times. It was noted that the share of doctors' salaries in the structure of all payments should be at least 50%.
In April 2025, Deputy Prime Minister Tatyana Golikova noted that the Russian Government would finalize the parameters of the new system of remuneration of medical workers by May 2025, and then transfer them to three regions for testing during the pilot project. Testing will take place in Yakutia, Kurgan and Lipetsk regions.
